Rows are appended and never edited. A reviewer who changes their mind adds a row that supersedes an earlier one and the earlier one stays, which is what makes this evidence rather than a status field. Each row's hash covers the row before it, so a change, a deletion or a reordering breaks the chain and this page says which row.
