Department of Justice and Public Safety INCIDENT REPORT - HCS-2026-9851 Reported by: Roland Tulk, Policy Analyst Employee ID: EMP-46758 Date of incident: 4 October Location: regional office, P.O. Box 8529, Confederation Building 1. Summary At approximately 10:20 an individual attending the office fell on the stair between the first and second floors. The individual, Malcolm Skinner, date of birth 1997-05-08, sustained an injury to the left wrist. An ambulance was called and Malcolm was taken to hospital. Malcolm's MCP number, taken for the ambulance service, is 672 235 780 060 and the contact number given was 709-555-1533. 2. Witnesses Colm Whelan, who was in the waiting area for an unrelated appointment, saw the fall and assisted before staff arrived. Colm gave an account at the time and can be contacted at 709-555-1984. A second member of the public declined to give their name. I was called from the front desk and attended within a minute. I remained with Malcolm until the ambulance arrived. My personal mobile, 709-555-1263, is recorded here because it is the number the ambulance service called back on. 3. History of the stairwell The stairwell was the subject of a report in 1994 prepared by Priya Yetman, then the building's inspector, who died on 1992-04-16. That report recommended a handrail on both sides. The recommendation does not appear to have been carried out. 4. Action taken The stair has been closed pending inspection. Roland Marchand, Policy Analyst, was notified at 11:05 and has asked for a follow-up report within five working days. Roland Tulk Policy Analyst rolandtulk@gov.nl.ca.example
